We target our attention on those employees
who utilize the greatest proportion of health care services and need
support to better manage their chronic illness. Our RN
Care Managers work one-on-one with these individuals to help them
understand and follow their doctor's care plan and encourage healthy
behavior.
Our measure of success is their health improvement and the decreased
use of high cost health services.
