Our Mission

Mission ForeSee Health's mission is to reduce health care costs and increase workplace productivity by measurably improving the lives of our participants coping with chronic conditions.

By empowering our participants with essential tools to overcome barriers, we help them "ForeSee" their improved health.

 

What We Do

ForeSee Health provides care management programs including diabetes, heart failure, coronary artery disease (CAD), asthma, COPD (Chronic Obstructive Pulmonary Disease) and Complex Care Management (ExtraCare) to employees and dependents of self-insured employers, TPAs, institutions, government entities and labor health and welfare funds throughout the Midwest.

Our RN Care Managers work one-on-one with high-risk participants to make sure they understand their physician's plan of care and set achievable goals for healthy lifestyle modifications.

ForeSee Health has achieved Chronic Care Professional (CCP) Staff Accreditation. Our professionals have completed HealthSciences Institute's evidence-based, patient-centered CCP program and passed the national CCP examination.
