ForeSee Health provides care management programs including Diabetes, Heart Failure, Asthma and COPD (Chronic Obstructive Pulmonary Disease) and Complex Care Management (ExtraCare) to employees and dependents of self-insured employers, TPAs, institutions, government entities and labor health and welfare funds throughout the Midwest.

Our RN Care Managers work one-on-one with high-risk participants to make sure they understand their physician's plan of care and set achievable goals for healthy lifestyle modifications.
