Brandon, Age 2 – Asthma
My two-year-old son has asthma and has been to the emergency room
several times this year with asthma attacks. My husband and I were
scared and didn’t know what to do to help our baby. Our care manager at
ForeSee Health taught us how to recognize the warning signs of an attack, and
even pointed out things we could do at home to help prevent further
attacks. Our Care Manager is working with us in a team effort—and we
feel like we’re better prepared to keep our son healthy.

John, Age 40 – Heart Problem

ForeSee Health nurse called me months ago, but I wasn’t feeling ill and
didn’t really feel I needed the program, so I said no thanks. But after
I went to the ER, they called me again, and this time I agreed to
participate in the program. When I talked with my nurse Kathy, she asked
a lot of questions about my health. She was interested in me and easy to
talk to! I mentioned that I had been more tired than usual and had some
indigestion. She told me that these symptoms could mean trouble with my
heart, to go to the ER if they got worse, and call my doctor first thing
in the morning. Well, they got worse later that evening, and I did go
the ER. This time they found I had 90% blockage in an artery that fed my
heart and I received a stent. I am fine now, but I could have had a
heart attack or worse. I look forward to working with Kathy to keep my
heart healthy!

Heidi, Age 54 – Diabetes
A year ago, I was not healthy—both physically and mentally. I had
high blood pressure, diabetes, and was overweight. I was also
depressed. I just wasn’t taking care of myself, and it was showing.
I wasn’t even seeing an endocrinologist, which is important to help
control my diabetes. My ForeSee Health Care Manager called me twice a week until I
agreed to see a doctor. He started me on medication to control my
blood pressure and depression, and changed my diabetes medication.
Now, I am looking forward to each day. My blood pressure is down, my
diabetes is under control, and I’m not depressed for the first time
in months. My Care Manager worked with my physician and me to get
results, and I feel much better.

William, Age 60 –
Diabetes
Before
I started working with my ForeSee Health Care Manager, I was overweight,
not active, and having a hard time controlling my diabetes. Her
advice gave me the encouragement I needed to start an exercise
program, and I decided to join the YMCA. It’s been twelve weeks, and
I’ve lost 18 pounds. I’ve even started following a healthy eating
plan. I’m really proud, because I’ve also gotten my A1C levels under
control. I finally feel like I am in charge of my diabetes.
