The high-risk participant’s clinical and functional status is reassessed on a regular basis as part of ForeSee’s commitment to continuous quality. The purpose of the reassessment is to ensure the care plan is effective and the participant is achieving his or her healthcare goals. If additional support or resources are needed, the RN Care Manager works with the participant to obtain them, and the care plan is modified as necessary to reflect the participant’s progress. Participants who have achieved improved control of their condition will be re-stratified to a lower acuity level. Participants who have not achieved improved control will receive additional services, including specialty physician referrals as needed and additional educational services from the RN Care Manager.
