ForeSee Health understands the essential relationship between physician and patient. We also understand that the complexities of high risk patients make it difficult for already busy physicians to keep up with the issues and problems faced by their patients. ForeSee Health RN Care Managers act as an extension of physicians in supporting their relationship with the high risk patient by working to improve patient compliance and encouraging healthy lifestyle modifications.

We communicate with physicians when their patients are identified for program enrollment and provide ongoing feedback and reporting of program outcomes, interventions and patient satisfaction.
